Advanced lipids: ApoB + Lp(a)

Count atherogenic particles, identify inherited Lp(a) risk and put cholesterol in context.

A standard lipid panel remains useful. ApoB estimates the number of atherogenic particles; Lp(a) is largely inherited and is generally measured at least once in adulthood under current U.S. guidance.

MEN, WOMEN + LIFE-STAGE CONTEXT

The correct range is personal context—not pink versus blue numbers.

Where biology differs, we call it out. Clinical interpretation should also reflect organs present, hormone therapy, pregnancy, age, medications and health history.

Women

Pregnancy and menopause can shift lipids; reproductive history can also inform overall cardiovascular risk.

Men

Risk often appears earlier on average, so age, blood pressure, smoking, family history and diabetes remain essential context.

The best assay cannot rescue the wrong timing, wrong specimen or wrong question. Follow the ordering clinician and laboratory instructions when they differ from this general guide.
01Fasting

Often unnecessary, but a fasting sample can help when triglycerides are high or the clinician requests it.

02Stable conditions

Avoid testing during an acute illness when possible.

03Units

Lp(a) may be reported in nmol/L or mg/dL; they are not reliably interchangeable with a single conversion.

REPEAT CADENCELp(a) is usually once in adulthood unless a specific clinical reason exists. Lipids and ApoB are repeated when risk or treatment changes.
