Private launch notice: Before patient invitations begin, the practice must finalize its legal privacy notice, retention schedule, practitioner access list, vendor agreements, incident-response process and applicable HIPAA/state-law review.
What we collect
Account identity, contact and demographic details; health goals and history; medications, supplements and allergies; lifestyle context; uploaded health documents; review requests; practitioner guidance; and a limited audit history of important record actions.
Why we use it
To organize the context you choose to provide, make it available to an authorized practitioner for a review you request, return guidance to your portal, protect access and let you export or delete your record.
Who can access it
Your signed-in account can access your member record. Only separately allowlisted practitioner accounts can open the review console. Important actions—including profile changes, uploads, file views and review updates—are recorded in an audit log.
Files & security
Files are stored separately from profile metadata and are delivered through authenticated routes, not public links. Accepted uploads are restricted by type and size. No internet service can promise absolute security; do not upload information unrelated to the review.
Your choices
- Choose which optional profile fields to complete.
- Consent separately to document storage and practitioner review.
- Download a portable JSON copy of your record.
- Delete individual files or permanently remove the full record.
Email & offers
Brief and pilot signups store your email, signup source, subscription status, consent version and consent timestamps. Permission to receive editorial or pilot email is required to join. Promotional email is a separate, optional choice. We may send promotions, partner offers or product updates only to subscribers who select that option. You can unsubscribe or change preferences at any time.
Medical boundaries
This portal is not an emergency service and is not continuously monitored. Portal content and guidance are educational and do not independently establish a diagnosis, prescription or clinician-patient relationship. Seek urgent care for severe or sudden symptoms.
Retention & contact
The practice’s final retention period and privacy contact will be published before public patient onboarding. Until then, this portal remains in a private implementation phase.
